Application Forms

The ACROD Parking Program updates permit application forms on an annual basis and is no longer accepting permit applications completed on old forms. To ensure you provide patients with a current ACROD Parking Program application form please download the form when needed and save our website to your list of ‘favourites’.

Eligibility Criteria

To be eligible for a permit, applicants must:

  • have a severe mobility impairment where walking more than 50m causes the physical condition to deteriorate and close proximity parking is required; and/or
  • require the use of a wheelchair, crutches, walking frame, calipers, or other specific mobility aid and require a wide parking bay to transfer in/out of a vehicle.

If your patient does not meet this criteria, they are not eligible for an Australian Disability Parking Permit.

Episodic Conditions: If your patient has an episodic condition you should contact the ACROD Parking Program on 9242 5544 to discuss their eligibility.

Types of Permits:

A temporary permit may be issued to applicants if:

  • the physical restrictions will improve;
  • future surgery is planned to alleviate or improve the mobility problem; or
  • it is too early to predict if the mobility restriction is permanent.

A two-year renewable permit may be issued if the mobility restrictions will:

  • remain the same; or
  • degenerate.

Auditing

All applications must be endorsed by a Doctor or Occupational Therapist and are audited by the ACROD Parking Program Occupational Therapist. To assist in processing your patient’s application efficiently, please provide us with as much information as possible regarding the applicant’s mobility. Please note that all applicants must pay a $16.50 administration fee, which is non-refundable if the application is declined.
